Deficient Practice Statement During the tour of the home, LPA observed an excer-saucer in the outdoor backyard area. Licensee stated that it is utilized when the children go outside for playtime. Deficient Practice Statement During the tour of the outdoor area, LPA observed a wooden gate standing on it's side, separating the left side yard from the play area for the children. This wooden gate was standing and kept in place with multiple long bricks standing up on both, the longer and shorter sides of it. LPA placed their hand on the gate and the gate easily swayed. The bricks as well as the unstable gate both pose an immediate health, safety or personal rights risk to persons in care. POC Due Date: 07/30/2024
