  Deficient Practice Statement Based on observation and interview, the licensee did not comply with the section cited above in that there were 2 out of 3 infants present (Child 4 [C4] & Child 5 [C5]) that did not have 15 minute safe sleep logs completed. Additionally, Child 1 (C1)'s last 15 minute safe sleep log was completed in June of 2025 and has no other more recent 15 minute safe sleep logs available to review, which poses/posed a potential health, safety or personal rights risk to persons in care.   102417(a) · The licensee shall be present in the home and shall ensure that children in care are supervised at all times [...] the licensee shall arrange for a substitute adult to care for and supervise the children during his/her absence.

  Licensee agrees to provide Acknowledgement of Receipt of Licensing Reports (LIC9224) to authorized representatives of all children enrolled and agrees to submit a written statement of understanding regarding the cited regulation. Based on interviews, the licensee did not comply with the section cited above in that Licensee confirmed she was unaware of time and place of the incident between two day care children that involved inappropriate behavior, which posed an immediate health and safety risk to persons in care. Licensee agrees to submit proof of written statement and completed LIC9224s for all children enrolled to Community Care Licensing (CCL) by the due date of 03/29/2025.
