D1 stated they will provide written plan, training materials, and sign-in sheet for training participants to LPA by email on or before POC due date. LPA observation of multiple children not provided with visual supervision for several minutes, occuring multiple times within a period of 40 minutes in the classroom. Information was received from facility that Staff #1 squeezed and scratched Child #1’s hand. Child #1 Assistant Director stated Staff #1 has been fired and LPA did not observe them on the premises. LPA advised that it is best practice to have more than one staff in a classroom to ensure accountability. Assistant Director stated she will give staff two scenarios and they will have to write their answer of what they would do. confirmed this incident occurred. Consistent statements during interviews revealed Staff #1 hit/slapped the cheek of Child #1 Assistant Director stated that a personal rights training was recently conducted, however she stated she will have staff watch the Personal Rights video on CCL's website: CCLD.CA.GOV and write a paragraph on what they learned. Assistant Director will have staff sign a paper that they watched the video and submit to LPA by COB Monday and the paragraphs once clompleted. Assistant Director stated she will speak to District Leader to come up with a long term plan.
