Staff training was conducted immediately following the incident in order to prevent this type of situation in the future. LPA reviewed memo that staff received. LPA requested that the memo was signed by each staff and submitted to LPA via email by POC due date. An Unusual Incident Report received from the facility stated that a child who was not supposed to ingest milk was given milk by a staff member. LPA observed a preschool classroom transitioning to nap time with 13 children and one staff. All children were still awake Director stated if they have extra staff they will send someone in to assist getting children to sleep. Director stated she will create a staff memo letting staff know that they cannot leave for lunch until all children are napping and staff will sign the memo. Director shall submit staff memo to LPA via email by POC due date. This is an immediate health and safety risk to children in care.
