Licensee stated after incident, facility has been printing their roster per classroom daily, and staff does a name to face check on the rosters, and submit the rosters to administration at the end of the day. Licensee stated this is done at every transition. Licensee provided copies to LPA. Deficiency cleared at time of visit.
Based on multiple witness statements and interviews, a child was left in the infant/toddler playground when the group and staff responsible for the child went back to their classroom from outdoor play and according to Director, child was out of sight for approximately two minutes, and another staff member from adjacent preschool playground saw the child and brought the child back to their specific group, which poses an immediate health, safety, or personal rights risk to persons in care

Based on LPA interviews, LPA learned of an incident at the facility in May 2025, when a child was left in the infant/toddler playground and did not have direct visual observation and supervision by a staff person. According to Director, incident was approximately two minutes.
