Representative also stated they will develop a detailed written plan for ensuring this does not occur again and will email the plan to LPA by close of business POC due date: jennifer.velasco@dss.ca.gov Facility self reported that on 06/26/2024, during a transition from the classroom to the playground, Child1 was left unattended in the Octopus classroom for approximately 17 minutes. Licensee stated the two responsible staff were terminated, parents and staff were immediately notified, and remaining staff will receive additional training on supervision during transitions. Facility self reported that on 11/01/2023, during afternoon transition from outside to inside, Child1 was left unattended on the playground for approximately two minutes. Staff #1 held Child #1's arms behind them, restraining them while telling Child #2 to hit Child #1 Director stated that following the Licensing visit that Staff #1 will be terminated effective immediately. Director stated she will give staff two scenarios and they will have to write their answer of what they would do. The documents will be signed and sent in to LPA Ferrara by POC due date.
