On 11/06/25 L1 sent a video via text message to LPA Strother of the outdoor play area, demonstrating that all of the black box style rodent traps with poison had been removed from the play yard. Based on observation, record review and interview, multiple rodent poison bait traps were observed on the preschool playground when the playground was used by children on 11/04/25, which posed a potential health and safety risk to children in care. On 11/17/25 L1 emailed LPA Strother stating that the bait boxes had been removed from the playground and that pest-control supplies will be stored in an area that is completely inaccessible to children and kept securely locked at all times.

L1 stated she will train all staff how to operate the thermostat and and make them aware of the temperature that is required in the classroom. During today's visit L1 completed a criminal background clearance transfer request form LIC9182, for S1, sending it to the Santa Rosa Regional office. S1 is now associated to the facility. L1 will submit a written procedure for hiring new staff, with steps to ensure each staff has an eligible clearance or exemption prior to working at the facility by 11/06/25.
