Based on interviews conducted 2 of 4 staff and confidential interviews revealed C1 was observed to be left alone in the office on 4/4/2025. It was not disclosed the length of time, but it was stated that multiple times C1 was left alone so the Director could prepare lunch in the kitchen and grab a child off the bus from outside. This is a potential risk to the health & safety of children in care.

Director stated all faucets/fountains have been replaced and facility is in the process of scheduling retesting. Director will submit new facility sketch, LIC 9275, LIC 9276 and results of retests to LPA Hamilton. LPA Hamilton received water testing results taken on 10/08/2022 with an Action Level Exceedance which is higher than the allowable limit. The faucet is located in school-age classroom.
