Facility will provide additional training to staff on responsibility for providing care and supervision as per Title 22 regulations and submit copies of the training to the department. Facility will also ensure the door to classroom B3 is in proper working order and can be opened by both staff and students. Director will provide video evidence to LPA via email. Based on video footage and interviews with staff, C1 was left unsupervised in the classroom B3 for approximately 4 minues, which posed a health and safety risk for children in care.
