Deficient Practice Statement Based on interview and observation the licensee did not comply with the section cited above in 1/1 play structures on the shared playground has more than 10 holes and/or breaks in the plastic throughout the base of the structure that connects to the rubber mat. Additionally more than 3 areas, approx larger than 14inx14in, once stepping onto the base of the structure, are covered with wood that is in disrepair and splintering. Plastic mat that covers this wood is peeling off of the wood, which all pose a potential health, safety or personal rights risk to persons in care. POC Due Date: 08/16/2024
