  101212(D1)(D2) · 101212 - Reporting Requirements-Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. The director will ensure that two or more cases of head lice or any other communicable disease is reported to CCL (Community Care Licensing). The director will submit a LIC 624 to the Department as soon as possible. This was not met based on evidence, the director did not inform the Department of two or more cases of Head Lice in September and October 2025. Based on Title 22 Regulations, two or more cases must be called within 24 hours and a written report (LIC 624) must be submitted within 7 days.
