  101216(g)(1) · Personnel Requirements (1) Except as specified in (3) below, good physical health shall be verified by a health screening, including a test for tuberculosis, performed by or under the supervision of a physician not more than one year prior to or seven days after employment or licensure.

  Deficient Practice Statement Based on LPA's observation and record review, two staff did not have a health screeing report in the file signed by a licensed physician. The licensee did not comply with the section cited above in that two staff records were incomplete. This poses/posed a potential health, safety or personal rights risk to persons in care. The director will submit a Plan of Correction to the Department by 6/26/23. The director will have a staff meeting about personal rights training and submit the agenda signed by staff to the Department within 30 days. The director will have the staff review the Department videos on Personal Rights. The director will submit LIC 624 (unusual incident report) This is evidenced by: Child #1 was not treated with dignity by a staff member. This is an immediate risk to the health and safety of children in care.

Disposition: Substantiated
