101229(a)(1) · Responsibility for Providing Care and Supervision. The licensee shall provide care and supervision as necessary...No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. Director stated staff were trained on 10/22/2025 in transitions and will conduct additional training on the three steps counting procedure to ensure all children are accounted for.

Based on interview and record review, the licensee did not comply with the section cited above in that it was determined that on 10/22/225 C1 was left unsupervised on the preschool playground for approximately 25 seconds, which poses a potential health and safety risk for children in care. Director stated a copy of the written transition procedure along with the sign in sheet will be reviewed and submitted to the San Diego Regional Office by 12/12/2025.
