The director emailed the LPA a copy of the power point, sign-in sheet from the meeting/training with staff for Active Supervision, and handouts provided to the staff. The director will email the LPA a summary from the 2 staff from Classroom #6 no later than 03/13/2024. Based on record review, interviews and video footage, the facility staff did not comply with the section cited above, as a child was left unsupervised for 8 minutes in the facility restroom during the transition to the playground.
