… Fence was fixed today while LPA was present. Director understands that she must check on fence periodcally to make sure planks are attached and fence is in safe condition. Based upon facility interviews with multiple parties, the facility did not fix the broken fence upon notification which poses a potential health, safety and personal rights risk to children in care.

Licensee states that the child was disenrolled by the parent and will be appealing the deficiency. Licensee states conditions of the Admission Agreement are met and will continue to be met. Based on interviews and file reviews, the facility disenrolled Child #1 based on an issue with the child's parent, not an issue with the child. This does not meet the conditions stated in the signed Admission Agreement and is a potential risk to the health and safety of children in care.

Licensee states that she disagrees with the deficiency and that access to the file and requested copies of documents were provided. Licensee will be appealing the citation. Based upon interviews, the facililty, denied the parent access to Child #1's file upon disenrollment. This is a potential risk to the health and safety of children in care.

The facility did not report a power outage that occurred on 10/28/22, lasting from approximately noon to closing which could pose a threat to the health and safety of children in care should the power outage cause disruption in care or the inability to provide and meet the needs of all children during the outage.
