CORRECTED IMMEDIATELY. Mr. Hamdard stated that he will return the bassinet to the parent who brought it in. Mrs. Hamdard removed the bassinet from the daycare area. She has a play pen that was stored in one of the bedrooms that she took out to be used by sleeping infant. Based on observation, Mrs. Fatima uses a bassinet for the infant in care. This bassinet has a lose fitting mattress and a cover on top. 102425(c) · INFANT SAFE SLEEP An Individual Infant Sleeping Plan [LIC 9227 (3/20)] shall be completed for each infant up to 12 month of age the provider has in care and maintained at the facility in the infant’s file.

Mr. Hamdard stated that he will help wife obtain completed safe sleep plan from the parents and submit to the department no later than 3/25/2025. Based on file review, licensee did not maintained infant safe sleep plan for two children (under one year old) on file.
