Per director, they will work on a plan to implenet more security and awareness when it comes to supervision of the children around the classroom pets. A meeting will be held about this topic with all staff. Director stated she would send a copy of the agenda to LPA Macias by 11/17/2025. Based on Director and parent interviews, it was determined that due to S1 lack of supervision, child had the time to reach the hot lamp on top of a Bearded Drangon's tank and burn his left hand. An emergency meeting was held on 05/19/2025 with all staff were they revised their Ratio/Name to Face, Break Board, and Emergency Drill protocols, and went over changes. All staff Signed a Sign in Sheet during meeting. LPA Adriana Macias was given a copy during inspection. Based on the information provided by the Director Kate Yohe, a 5 year old child was left alone and unattended on the playground for approximatley 15 minutes by S1 and S2 on 05/16/2025. Ms. Yohe met with her staff on 12/20/23 to discuss supervision of children to prevent incident from occurring again. Facility installed a spring to the gate entrance to keep gate closed at all times. Based on the information provided by site director Kate Yohe, a 2-year old child wandered away from the facility without supervision from staff. Facility will also install a door alarm to sound off whenever the door is opened.
