101212(d) · 101212 Reporting Requirements. Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department.... The Director, Jessica Williams, stated she did not fully understand this regulation. LPA explained the regulation fully at the time of inspection. Miss Williams stated that she will make sure to review the reporting requirement regulation and ensure that anything unusual is reported to the Department within 24 hours. Based on file review, staff interviews, and information gathered during another matter, the facility did not report to the Department about an incident that occurred in August 2025, that involved a staff member (S7) mishandling a child (C1), which poses/posed a potential health, safety, or personal rights risk to children in care. The Center Director stated she will conduct a training and submit agenda and list of attendees to the Dept by 5/23/25. Based on staff interviews conducted by LPA, several staff stated they have witnessed a staff member make rude comments about children and behave unfriendly towards parents. 101229(1)(1) · ( Responsibility for Providing Care and Supervision. The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation.
Director states that the mirror will be adjusted to ensure that all areas of the classroom are visible from the changing area. In the meantime, the other children will be kept in an area, closer to the changing table where the staff can visibly observe children at all times. Based on observation, interview and review of documentation, Child #1 was unobserved for a brief period while Staff #1 was changing another child's diaper during which time Child #1 was bitten on the arm. As the period of time was short and no medical attention was required, this is a potential risk to the health and safety of children in care.
