Director stated she is conducting a staff training on personal rights today 5/9/25 and will submit proof of agenda and staff signature sheet to LPA on 5/12/25 via e-mail. Based upon video review and interviews with staff and children, S1 pushed C1 causing child to fall down which is a potential health, safety and personal rights risk to children in care. Assistant director stated that training was provided with all staff members on 3/4/22 regarding Children's Rights Violation- Zero Tolerance. A copy of the Agenda, training, and sign-in sheet was obtained today. Based on information obtained from the facility, records reviewed and interviews conducted, a staff person violated the Personal Rights of a child by placing her foot on a child’s neck on 2/17/22.
