This requirement was not as evidenced by Staff#1 and Staff#2 being observed to be in a classroom 22 children on 05/05/22. Staff#1 did not have any units on file. Although Staff#1 was removed The facility removed Staff#1 from the classroom and replaced with Staff#3 who had enough units for the classroom to be in ratio on 05/05/22. and replaced with Staff#3 so the classroom would be in proper ratio. This was a potential risk to children in care.

Director shall submit a detailed staff schedule outlining staff room assignments and coverage. Director will conduct a training on teacher-child ratios and provide a copy of the sign-in sheet and training agenda by the POC due date of 07/23/2021. Based on interviews conducted and records reviewed, the Director did not ensure that the facility maintain teacher to child ratio.
