Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above. During today's inspection, LPA observed three upright, metal bolts cemented in the fenced and a sheared off metal post with pointed parts in backyard area which poses a potential health, safety or personal rights risk to persons in care. Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above. LPA observed cleaning compounds in under the kitchen sink and in the laundry room that were accessible to children in care. LPA also observed hygiene items that must be kept out of reach of children accessible in the bathroom drawers which posed a potential health, safety or personal rights risk to persons in care.
