  Facility will began conducting physical and visual sweeps of all outside areas prior to children and submit a signed checklist to LPA for one full week beginning 1/15/26 and continue process to ensure safety of children. Based on interviews, the facility did not comply with the section cited above where a child was able to obtain a vape pen without visual supervision of a teacher, which poses an immediate health, safety or personal rights risk to persons in care.
