Site Director conducted one on one training with all staff in the infant program regarding high chair safety, standard infant room operating procedures, supervision, feeding, and communication. The high chairs were replaced with floor high chairs. Deficiency corrected. Based on interviews and available evidence, a child (C-1) fell from the high chair when a staff (S-1) removed the tray attached to the high chair and turned away to place the tray in the sink. This poses an immediate risk to the health, safety, and personal rights of children in care. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.
