This regulation was not met as evidenced by Principal states they have updated policy and trained staff to maintain all doors are completely closed requiring a key to open no longer being held ajar by magnet. LPA observed doors to be completely closed at time of inspection with key by door and sign in/out for key. Clearing deficiency. Based on LPA observation and interview, Licensee did not have full supervision of child resulting in child leaving classroom for a period of 5-10 minutes and entering elementary school classroom. Based on record review the licensee did not ensure water sources at facility meet lead requirements. Lead test results showed that 10 water sources have an action level exceedance.
