Director stated they will make sure they report any outbreaks of 2 or more cases within 24 hours via phone call and submit a written incident report within 7 days. Director stated they will also notify families of any outbreaks. Director will submit LIC624 Unusual incident report for the last lice outbreak by 9/30/24.
Based on interview with Staff#11 it was determined that S11 did not report the lice outbreak when there was 2 or more cases in the childcare center to families, licensing and department of public health. This is a potential Health and Safety risk to the children in care

(a) The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons. The director is going to meet with the Staff#6 today and will provide training to Staff#6 and then follow up in a couple weeks to see what she learn from the training and come up with an action plan to how to improve her skills in communication. The director will also provide a written statement and will provide proof to LPA by the POC due date.
Based on LPA’s interviews, 3 staff witnessed S6 yelling at day care children at the facility. This action poses an immediate risk to the health and safety to the child in care

as evidenced Licensee had labeled, covered and taped the 3 outlets. Licensee is utilizing the other faucets as source of water, also using personal water bottles as source of drinking water. Newport Mesa School District plan of action is to replace fixtures where high levels of lead were detected. Director to send LPA plan/resolution by poc date by LPAs review of facilitiy records. Based on facility record review, it was discovered that outlet iln Rm 14, Rm19 and Kitchen Prep had a high level of lead. This poses a potential risk to the health of children in care.
