Director Martha Prysock, told LPA that they had already implemented new bathroom supervision procedures. She will ensure to provide training and written supervision procedures to all preschool staff. Director will email copy of the attendance signing sheet and new written supervision procedures to the San Diego Child Care Regional Office (SDCCRO) Based on LPA observations and interviews conducted, the licensee did not comply with the section cited above, as staff did not maintain visual observation of children inside the restroom, which poses an immediate Health and Safety risk to children in care.

The Pastor stated he will provide staff with a Covid-19 ouch report for staff to document any Covid-19 like symptoms. The Pastor stated the ouch report will have an area for parent contact details. This POC will be sent to LPA no later than 11/19/2021. Based on observations and interviews, staff stated five children were in care displaying a runny nose. On 10/21/2021, LPA observed 14 children and one staff without a face covering in classroom #2. LPA observed a child in care displaying a runny nose. On 10/27/2021, LPA and LPM observed three children in care displaying a runny nose. Staff were observed with face coverings below the nose or under the chin. This poses an immediate health, safety and personal rights risk to children in care.

The Pastor stated he will create a letter regarding children's rights. The Pastor will provide new and current staff a copy of the letter along with a staff signature. The POC will be sent to the LPA no later than 12/16/2021. Based on undetermined dates, at least two staff observed two staff telling children they were busy in a demeaning and disrespectful voice tone. Another staff was heard making humiliating comments towards a child in care. This poses a potential Health and Safety risk to the children in care.

The pastor stated he will provide LPA a revised letter to the daycare parents regarding the face covering policy. The pastor will submit a written face covering letter, signed and dated by each staff no later than 11/19/2021.
