LPA discussed supervision of children in care with Licensee. Licensee provided written confirmation that S1 was immediately terminated on 09/18/2025. Licensee also provided a written plan of action during the visit regarding supervision to ensure children in care are supervised at all times. Based on observations and evidence collected, it was revealed that S1 did not provide adequate supervision of all children in care resulting in C1 wandering into the street which poses an immediate safety risk to children in care. An office visit may be scheduled by management to discuss this issue.

During facility tour the LPA observed stairs that were not barricaded with two children present under the age of five.
