Deficient Practice Statement Based on observation, interview and record review, the licensee did not comply with the section cited above in 2 staff present and supervising children did not have proof of the LIC503 health screening, and 1 staff did not have proof of a screening for tuberculosis on file, which poses a potential health, safety or personal rights risk to persons in care. Upon arrival the LPA was greeted by Jennette who stated she is the Director and has been the Director for six years. as eveidenced by Facility will post additonal signs at facility for mask wearing and implement CDPH face mask guidance including email notification to parents by POC due date 12/22/21. Facility will send pictures of addtional signs, letter to parents to LPA Carbullido at giselle.carbullido@dss.ca.gov. Based on LPA observation and staff interviews, the facility is not following COVID-19 best practices for mask wearing by children. This poses a potential health and safety risk for children in care.
