  101223(a)(1) · (a) The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons. Based on the observation, interview, record review, the Licensee did not meet in place to prevent recurrence. Director understands the Department acknowledges required steps are being taken but also understands while those steps are being take, all children’s rights must be priority. Director agrees to submit a plan to the above regulation which poses a potential Personal Rights risk to the children in care. During the investigation, there were two or more known cases of the same inappropriate behavior challenges. Appropirate procedural steps were taken, however, there is no plan prevent recurrence in the classroom to the Department on or by POC due date of 4-24-2026.

  The facility will notify the health department, call the licensing department and submit written documentation per reporting requirements. Based on record view and interview the facility did not report the outbreak to the department or health department as required. This is a potential health and safety risk to children in care.
