101229(a)(1) · 101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. THE FACILITY HAS SINCE INSTALLED ALARMS ON ALL DOORS OF THE FACILITY, REPOSITIONED THE CHANGING TABLE IN THE CLASS ROOM SO STAFF IS ABLE TO OBSERVE THE ENTIRE ROOM WHILE CHANGING DIAPERS, NOTIFIED DAY CARE PARENTS OF THE INCIDENT VIA
INTERVIEWS AND OBSERVATION WHICH REVEALED A CHILD WAS ABLE TO WALK OUT OF THE CLASS ROOM AND BUILDING WHILE THE TEACHER'S BACK WAS TURNED WHILE CHANGING ANOTHER CHLD'S DIAPER LETTER AND HELD A MEETING WITH THE CHILD'S PARENTS. LICENSEE WILL ALSO HAVE AN ALL STAFF TRAINING ON CHILD SUPERVISION. LICENSEE WILL SUBMIT TO COMMUNITY CARE LICENSING A WRITTEN PLAN OF ACTION AND A LIST/SIGN IN SHEET OF ALL STAFF WHO ATTEND THE TRAINING BY 5/11/2023. SUPERVISOR'S NAME: Loretta Dyson

document review and staff interviews. Unusual incident report was reported to the office a month after the hand-foot-mouth began. Per staff interviews, cases were reported to be between 10-20 cases. This poses a potential health and safety risk to children in care Director, Amy will submit the follow-up written unusual incident report that indicates all the cases of hand-foot-mouth disease that ocurred in the facility. In addition, Director will watch the child care licensing video on reporting requirements and submit a statement to LPA on what it was about.
