LPA on 7/10/24 observed gate between the preschool and toddler play ground fixed and repiared. LPA requested ticket forms of play yards completion on what was done and on what dates the facility did repairs to be sent by POC due date. Interviews with 3 staff confirmed broken gate, approx. broken Jan/ Feb 24'. Parent interview with Parent #3 stated child enrolled in school parent helped fix gate and kept breaking. LPA's Caldron observed gate having a metal chain and not the latch that goes to gate.

LPA on 8/7/24 observed areas in preschool and toddler side fixed and repaired and areas were flat , leveled and no safety concerns observed. LPA requested ticket forms of play yards completion on what was done and on what dates the facility did repairs to be by 8/30/24 to LPA Calderon. Staff reporting and Staff #1 fell on the todder play yard due to floor having a dip (sunken in turf) and being uneven. LPA Calderon and LPA Sanchez observed unleveled floor, dips and holes in preschool side. Preschool side had holes in dirt area near fence, grass was unlevel near cement block and turf area.

LPAs observations and interviews conducted with staff and parents. On 05/21/24, LPAs observed and photographed, broken toys, door, and a All corrections were made the same week. LPAs observed corrections on 06/07/24. Corrections were cleared during todays inspection. a bent gate. Staff and parents interviewed stated that the gate between the preschool yard and toddler yard was also broken. This poses a potential risk to the health and safety of the children in care.

Based on Director stating that she threw away half the child's plate away and Director states that since this incident she apologized to the child. Plates are replaced right away when their broken. Monthly training have been provided to all staff about handling situations in a positive effective matter. Director will submit a written statement on how would have handled the situation did not offer child a new plate. This is a potential risk to the health and safety of the children in care. differently now that there has been more training's.
