Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above in LPAs observed in the outdoor area wooden door with missing bottom piece. The broken bottom piece of the door exposed wooden splinters and near the broken door was an unused water fountain with stagnant polluted water which poses/posed a potential health, safety or personal rights risk to persons in care.
