record review and interview Administrator Patrick Dodd removed staff #1 today from the preschool classroom and placed a Department cleared staff (staff #2) to stay within ratio. Today, staff #1 was sent to get the proper clearance and will not return to the facility until she/he receives proof of Department clearance. Staff #1 obtained criminal record clearance through his/her employer but not the Department of Social Services. According to records review, staff #1 does not have a CACI clearance. Administrator confirmed this information. A civil penalty of 100.00 was assessed today. Proof of stamped Live Scan form will be provided by POC date: 1/28/2022.
