evidenced by Director Nelson immediately made the soccer goal posts inaccessible to children by moving them away from the outdoor play area and storing the goal posts in the facility’s parking lot. Furthermore, Director Nelson will provide staff training regarding zoning and areas deemed as “hot spots” for accidents. Based on interviews and document review, Child #1 sustained an injury while in care that required medical attention. Per interviews, child #1 was hanging from the soccer goal nets and the goal posts flipped and landed on the child. Per interviews, the goal posts were not secured and anchored to the ground during the incident, which posed an immediate risk to the health, safety, and personal rights of children in care. Supervision shall include visual observation. Director Nelson provided a copy of the training that was conducted on 12/10/2024. The training was regarding active supervision and classroom transitions. Director Nelson stated that she will include the training for all new staff hired going forward.

Based on interviews, the licensee did not comply with the section cited above. During today's inspection LPA gathered information that staff left a child unattented for approximately 2 minutes which poses a potential health, safety or personal rights risk to persons in care Furthermore Director Beth Nelson will provide a statement as how she will implent the training during new hire onborading and monthly staff meetings.
