Based on interviews and document review, Child #1 received an injury while in care, and Child #1 expressed a concern to Teacher #1 which was not handled appropriately or reported to a supervisor or Director. This poses a potential risk to the health, safety, and personal rights of children in care. Director agreed to provide CCL with an outline of the training materials covered as well as a log of staff who attend the training by 11/05/2021.
