Director stated transitions are being monitored by management oversight and an all staff training will be conducted of supervision and transition procedures. Director will submit training material to LPA Marx by POC due date and will submit attendance signatures to LPA Marx once training was completed. Based on interview and record review, the licensee did not ensure compliance of the section cited above by leaving children outside without supervision, which poses an immediate Health, Safety, or Personal Rights risk to persons in care.
