The facility will strengthen dismissal supervision by assigning a staff member to monitor the gate daily and maintaining a supervision log. The director will conduct regular spot checks, and all staff—including substitutes and new hires—will complete refresher training on supervision protocols. Based on interviews and record review, it was determined that a child in care wandered off into the upper parking lot during dismissal due to a lack of supervision, which poses an immediate health, safety, or personal rights risk to children in care.
