  Director stated that they will mail a copy of the child’s record to their representative and submit a copy of the certified mail receipt to the San Diego Regional Office by 09/05/2025. Based on LPA interviews and record review, the facility did not comply with the section cited above in that they denied the child’s authorized representative the opportunity to inspect the child’s record, which poses a potential health, safety, and personal rights risk to children in care.
