  The Assistant Director stated they will conduct a transition and supervision training on 09/11/2025. Assistant Director will create a sign in sheet and have attendees sign the sheet, and submit a copy of the sheet to the San Diego Regional Office (SDRO) no later than 9/12/2025. Based on interviews and records reviewed, the licensee did not comply with the section cited above, as a child was left unattended on the playground for approximately four (4) minutes before being found by staff. This poses an immediate health, safety, and personal rights risk to children in care.
