The Director Stated they added restrooms to their brightwheel app in Early July 2025 and also installed a blackboards in the restrooms and staff must write down number of children coming in and erase as they leave. Director is monitoring the bathrooms to make sure that the correct number is written on the walls and director sends memos reminding the staff. Based on Director admittance, staff and witness interviews and video surveillance footage, the Child Care Center did not comply with the section cited above in that (C1) was left unattended in a bathroom without supervision which poses an immediate health, safety or personal rights risk to persons in care. Memo copy obtained. Checklist for new-hires under policy and procedures Director will add additional review/training for new staff regarding these new policy for documenting the children as they move about the facility by 10/01/2025.

The Director Stated she will review the regulation again with staff. Director stated she will create an accountability chart with training; a visual to help ensure incidents are reported to all necessary parties. Director stated she will send these created documents/trainings to SDRO by 10/1/2025. Based on interviews and record review the facility did not comply with the section cited above in that the Child's Authorized Representative was not informed regarding an incident that occured on 06/18/2025. Site Supervisor stated that the teacher whom had been responsible for the same violation on 8/9/2024, who had been trained as previously agreed was terminated. Director stated that she would like to participate in the CCLD Advocacy program. LPA will initiate the referral 02/25/2025. Based on interview and record review, the Licensee did not comply with the section cited above and that on 12/6/24, there was an absence of supervision resulting in C1 being left unsupervised on the playground which poses an immediate, safety, or personal rights risk to persons in care. Site Director stated they have switched to brightweel app which allows for the office staff and parents to see where the children are at in the facility; to help ensure staff accountability. This will be another level of name to face transition to ensure this will not happen again and pictures are being added.

Director and licensee will review personal rights of children, along with conducting a staff in-service training that addresses the topics of personal rights of children, and cell phone policies. Director will provide a written statement of these procedures, date, and agenda of training to LPA by 04/05/24. (b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. The facility has made outlets that tested with an ALE out of service and posted appropriate signage. Director stated they have replaced the impacted outlets. Fixtures were retested by a certified water sampler on 12/01/22, following the protocol outlined in Written Directives, Section 101705. Director stated that parents will be notified of the lead exceedances. Based on record review, the facility was found out of compliance with the above directive, as two (2) outlets not used for drinking and/or food preparation tested with an Action Level Exceedance (ALE). This poses a potential Health and Safety risk to children in care. Director stated the POC will be provided to the San Diego Regional Office no later than 01/07/2023.
