Based on interviews and records reviewed, a day care child displayed aggression towards other day care children on multiple incidents that occurred at the facility between 02/02/2024 through 01/13/2025, which is an immediate risk to the health and safety of children in care challenging behavior, setting behavior goals, and monitoring behavior progress (Part 1), and provide in-service training regarding documentation of incident reports, including proof of staff participation (Part 2).
