Based on interviews and records review, the licensee did not comply with the section cited above. In reviewing facility files, there was no proof that the facility informed the parent/legal Licensee has conducted meetings and training with the entire staff. Licensee has also implemented additional logs and reports to assure proper documentation and communication with Parents/Legal Guardian. Samples of logs and reports were provided as verification. guardian of any behavioral issues concerning their child(ren). Also no information/proof obtained that, if, there were any behavioral issues, what effort was made by the facility to rectify the situation before termination. This poses a potential health, safety, or personal rights risk to persons in care.

101216(i)(2) · Personnel Requirement - Request a transfer of a criminal record clearance as specified in Section 101170(f). This requirement was not being met as evidenced by the observation made by the LPA that staff Kathia Gonzales was providing care and supervision to children in care however, Licensee immediately submitted documentation to have staff associated to the facility. Licensee and/or Director will submit a statement regarding how the facility will assure compliance with the cited regulation. staff was/is not associated to the facility. This poses an immediate risk to the health, safety, and personal rights of the children in care.
