Facility agrees to conduct a training with all staff members regarding supervision of children. Proof (copies) of the trainng agenda and a dated sign in sheet with staff signatues will be submitted to the Department by the POC due date. Based on records review, interview, and self-report from the facility, the facility did not maintain visual observation of 1 child, for about 2 minutes, while the child was on the facility playground, with the playground door propped open. This poses/posed an immediate health, safety or personal rights risk to persons in care. Deficient Practice Statement Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the children’s file review the LPA was unable to review immunizations for S5. S5 had an exemption on file, however, the exemption is not valid due to it not being signed by a California Doctor. POC Due Date: 09/16/2025
