Facility will submit proof (copies) of a detailed plan on how they will prevent further instances of operating out of ratio, especially during the morning hours and transition times. This plan will include how & when the staff will contact management regarding ratio. Based on record reviews and interviews, the licensee did not comply with the section cited above in that there were at least 3 out of 3 randomly selected dates that showed multiple rooms where the facility was operating outside of the 1:12 teacher to preschool age child ratio for 5 minutes or more. This plan will be signed and dated by all staff members that work in the preschool age program. This will be submitted to the Department by the POC due date.

This is a repeat violation. The facility director agrees to submit a statement of understanding the Responsibility for Providing Care and Supervision at all times for all children in care. Based on evidence gathered through interviews, documentation obtained & the agency's self-reported notification that a child was left unsupervised on 10/31/2024 for 2-3 minutes, This is an immediate risk to the health and safety of children in care. A civil penalty was issued for $1,000.00 In addition, the director agrees to submit written plan to ensure 100% Supervision and steps to implement the plan of action, curriculum provided to staff, on or before the close of business day on 11/15/2024.
