101223(a) · Personal Rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; A copy of the agenda and sign in sheet for an in person meeting that was held on Oct 13, 2025, with all staff was provided during today's inspection. The agenda included the following topics: Personal Rights, Code of Conduct, and staff expectations in relation to the care of children. or withholding of shelter, clothing, medication or aids to physical functioning.

Information obtained that a staff would withhold milk or water from child(ren) as a form of discipline, tell child(ren) that they were not smart, and leaving children soiled while blaming them for doing so. Please note that staff denied any and all allgations. This poses an immediate health, safety or personal rights risk to persons in care.
