Deficient Practice Statement Based on observation At 2:30pm LPA Calderon observed in the back yard a loose gray cord by the door that leads to the children's play room and (2) loose cords, one black cord and white cord in the side runner that is accessible by children and located in the back yard children, pictures taken, the licensee did not comply with the section cited above in [1] out of [1] hazadous items which poses a potential health, safety or personal rights risk to persons in care.
