THIS IS AN AMENDED REPORT DELIVERED ON 6/02/2026 Director states that the alleged staff member has been transferred as of March 9, 2026. The director states she will submit a written Plan of Correction outlining the facility’s procedures to prevent future personal rights violations. The plan will include at minimum: Implemented corrective measures, staff observations, detailed reason for S4 transfer any training to be provided to S4, location of reassignment for S4, and steps to monitor behavior of S4, as well as provide training on personal rights to all current and future staff and provide a copy of training outline and sign in sheet to Department by Due date of 6/17/2026.
Based on observation and interviews, the licensee did not comply with the section cited above as a staff member was observed yelling and firmly holding a child which poses/posed a potential health, safety or personal rights risk to persons in care

Based on information gather during investigation interviews it is determined that the Director and Assistant Director (AD) have worked on their laptop computers and used the center phone when they are in the classrooms covering breaks or filling in for absent staff.

Director stated, C1 has been placed in an older children classroom for 2 weeks now where C1 has shown to adjust well without any incidents of harm to another person. Behavioral plan is in place to monitor, intervene, and remove C1 if necessary. Deficiency was cleared as of today's site visit. Based on interviews and documentation reviewed records between October 2022 through February 2024 show child C1 received multiple incident reports documenting harm to other children. Behavioral interventions did not adequately reduce harmful behaviors. Staff Interviews confirmed incident reports, C1’s behaviors, and lack of consistent effectiveness of interventions.

Director states that immediately after the incident she conducted one on one training on transtions with Staff #1 documenting what was talked about and having her sign she understands and has been monitoring transitions. On April 5th a formal staff meeting was conducted, covering transition procedures and proper supervision. Based on information obtained though interviews and review of documentation, Child #1 was left behind in the classroom, without supervision, for a period of approximately 2-3 minutes before being found by a staff member and taken back outside. This is a potential hazard to children in care. Director states that there will be another staff meeting on May 2nd where transitioning and supervision will be covered again. At that time she will show the CCLD "Supervising Children in Child Care Centers" to the staff from our website. Director states that right after the incident, teachers were immediately reminded of proper transitioning and observed their transition procedure. A formal training will be conducted on 2/21/22, addressing supervision and transition and incorporating CCLD video: "Supervising Children in Childcare Centers." A copy of agenda and the roster will be sent to Based on information obtained though interviews and review of documentation, Child #1 was left behind in the classroom, without supervision, while the class transitioned outdoors, for a period of approximately 18 minutes before being found and comforted by the Assistant Director Corrie Barrick. Director states that the transition process currently in place involves a face to name to line up at the door, a face to name and count as the children transition thru the door and a final count before they close the door.
