Director, Vanessa Milroy, stated she will be conducting a training about supervision on 06/25/2025, and then a follow up training on 06/27/2025. Director will provide the LPA the training agenda and sign in/out sheet with staff member signatures on it by 06/30/2025. Based on interviews with the Director, Vanessa Milroy, staff memebers (S1-S3) and documents obtained , an infant child was left alone inside a closet between Classroom 1 and 2 for approximatley 5 minutes without supervision on 06/13/2025. Director states staff meeting will all staff occurred 1/27/25 regarding safety supervion, name to face protocol, policies and procedures, door alarms and the use of walkie talkies for transition. Director provided agenda for this meeting dated 1/27/25. Based on interview with Director, Staff S1 did not ensure supervision of an infant which poses an immediate Health and Safety risks to persons in care. Director states that this water fountain has been made inaccessible as the water needed to be turned back on to complete the flushing and the fact that it was turned off since the pandemic started might have been the cause of the lead build up. Repairs have been started and Director will keep Licensing informed of the progress. Based on water testing results and interviews, facility tested over the Action Level Exceedance level at on water fountain in Room 2.
