Office Administrator states that S1 no longer works at the facility and Director conducted a staff training on children's personal rights and Forbidden Acts. LPA obtained a copy of staff training agenda and reviewed staff sign-in sheet confirming staff participation. Deficiency cleared at visit. Based on interviews and documentation, it was determined on at least 3 separate occasions, staff S1 violated children’s personal rights by grabbing their arms and swinging them in another direction, yanking them, and exhibiting other rough behaviors toward children which poses/posed an immediate health, safety or personal rights risk to children in care. Director stated she request to have the changing tables in the rooms 3 & 4 repositioned to have the capability for staff to observed the entire classroom when diaper changing table is in use and provided training video on “Supervising Children in Child Care Centers” at https://ccld.childcarevideos.org/child-care-center-operators/ . Based on investigation interviews and documentation children in the 2’s classroom have been left unsupervised in situations when a single staff is using the diaper changing table and unproperly supervised using the bathroom while they are potty training. Also, documentation determined child C1 was left unsupervised in a classroom and discovered by a staff which posed a potential health, safety or personal rights risk to children in care. Director will provide the department with sign-in sheet of all the staff that have reviewed the training and proof of repositioned table by 10/10/24.

Assist. Principal stated, incident occurred due to the staff invovled not following school's face-to-name policy/procedure on Tadpole system. All staff received refresher training on face-to-name procedure 2 days prior to incident. Staff invovled were terminated from emplyment. Deficiency is cleared during visit. Based on interviews and information gathered, child C1 was left unsupervised on the playground for approximately 20 minutes after the child’s class returned to their classroom. Director states that this water fountains have been made inaccessible as the water needed to be turned back on to complete the flushing and the fact that it was turned off since the pandemic started might have been the cause of the lead build up. The kitchen faucet was rarely used for food prep before or after Covid started because it had a low water flow. Based on water testing results and interviews, facility tested over the Action Level Exceedance level at a water fountain in Room 5 and 9 and a food prep sink faucet in the ktichen. This poses an immediate/potential health, safety or personal rights risk to children in care. Repairs have been started and Director will keep Licensing informed of the progress.
