Director stated, since the incident, all staff were provided a retraining on supervision of children and the staff members responsible recieved additional training, including but limited to, "Transition Tracking" with certificate of completion provided to the Department. Deficiency cleared by site visit. Based on interviews and information gathered, C1 was unsupervised for 1-5 minutes in a classroom after using the bathroom. There was a miscommunication between 2 staff where they thought the other staff was going to remain in the room to supervise the child.
